I need to get as many people as possible to take this survey for the research methods course I am currently taking. The study I am conducting is researching the relationship between Age and Overall Wellness. Your participation will be greatly appreciated.

The Relationship Between Age and Overall Wellness


You are being asked to participate in a research study investigating the relationship between Age and overall wellness.

You have been selected to participate because individuals that access the internet fall into many different age groups.

This study is being conducted by Christine Watson, a student at ASU and MCC, as part of the requirements for a Research Methods course in Psychology. It is being conducted with the permission of Mesa Community College and MCC’s Psychology Department (480.461.7032), under the guidance of Dr. Ed Lipinski (lipinski@mail.mc.maricopa.edu).

If you agree to participate, you will be asked to complete a survey relating to your overall wellness. Additionally, you will be asked to provide some demographic information (i.e. age, gender, etc.). It will take you approximately 10 minutes to complete the survey. You will receive of copy of this form for your records.

Your participation is voluntary. If at any time during this study you wish to withdraw your participation, you are free to do so without any consequence. Additionally, your responses will not be associated with you in any way and all collected data will be kept in the strictest confidence; only anonymous group results will be reported.

AUTHORIZATION: By filling out and submitting this online survey, I agree that I am at least 18 years of age, that I have read the above and understand the nature of the study, that my participation is voluntary and that I may withdraw at any time without consequence. Furthermore, I may contact the researcher (Christine Watson, christine.watson@asu.edu) with any questions that I might have about the study or contact MCC or Dr. Lipinski (as listed above) with any questions or concerns related to my participation in the study.
